| Business information | Business name, entity type, address, EIN, start date, industry, time in business, and a short description. |

| Owner and contact information | Your name, mobile phone, email, home address, date of birth, and ownership percentage, plus a second owner’s details if applicable. |
| Financial information | Approximate monthly revenue, the amount requested, and recent business bank statements. |
| Identity information | Your Social Security Number, collected only at the application’s final, clearly labeled step with your separate consent. |
| Business location | Applications are currently accepted from businesses in all 50 states and DC. |
